We require a notice of 48 hours in the event that you need to reschedule your appointment.
Payment is due at the end of the appointment. We accept Debit, Visa, and Mastercard.
Reimbursement will be issued directly to you if you have any insurance coverage. We will be gladly assist you by completing your insurance forms, including any pre-authorizations. Your insurance company will pay your portion directly to you.
